The following is a shell for a TMR. Those sections of the TMR that are "canned" (at least for the time being), are filled in (in regular typeface). Every effort has been made to include all the possible slots; not all of these will be used each time. Brackets ([ ]) contain explanatory notes, and do not appear in an actual TMR. Numbers in parentheses reference notes in
